Office Hours:  Appointments are available Mondays through Thursdays 9:00 A.M. - 9:00 P.M. visit.

Timeliness:  Due to the Therapist's full schedule, we ask that you arrive at least 15 minutes prior to your appointment time, and come prepared with your patient intake forms,insurance information ready. Patient Intake and Consent Forms can be downloaded online here.

Charges:  All fees are payable at the time of the visit. We accept MasterCard, Visa, Am Express, checks, and cash. We accept Cigna/United Health/ Blue Shield PPOs, Tri Care/ VA Healthcare insurances and you must have a valid card to present at your scheduled appointment, otherwise a payment by credit card, check, or cash will be required. Please call to discuss other insurance plans.

Fee Policy: All patients will be required to have a credit card held on file to make payments for insurance benefits denied, deductibles not met, inaccurate copay payments, and cancellations with less than 24 hours notice.
